What to bring for games

2008
09.16

So here is the paperwork for each game:

* Each coach gets a packet that comes from SYSA and goes to each club registrar
* there are smaller sheets with carbon copy pages to be filled out for the ref for the games.
* Let’s see for reporting scores…the ref is suppose to report the scores. If no ref was there then you need to call the score in. The number and e-mail are at www.sysa.org under standings.

You should also bring your signed roster showing that everyone is age verified and all the medical forms in case someone is hurt.

Medical Waiver Forms are handled by coaches

2008
09.03

We don’t mess with the medical waiver forms. The team managers/coaches hold on to these. They can print them out all together at the team page or players can print them out and get them to their team manager. Sometimes I will print the whole batch out for a team if they can’t seem to get them. Every player is suppose to get them by e-mail when they register. This doesn’t always happen. If a player needs to retrieve the form they can do it at www.capitolhillsoccer.org then log-in using the button in the upper left hand corner then click on past registrations then click on print forms for the appropriate registration year (2008 now)
